Gross lesions of interstitial pneumonia and enlarged lymph nodes due to PRRS infection may be observed. However, these lesions can be observed during other infections; as previously mentioned, there is neither pathognomonic gross lesion in lungs from pigs suffering PRRS respiratory outbreaks nor in sows or aborted foetuses from PRRS reproductive failures.

Microscopically, interstitial pneumonia is the primary lesion, but it is nonspecific for PRRS virus.
